If you are organizing an event and would like the Mayor and/or Council Member(s) to attend please complete the form below. It is essential that the Mayor's Office be informed at least ten (10) days prior to a function if the Mayor and/or Council Member(s) are required to speak. Adequate details of the subject, and any other relevant information should be provided. If the function is a luncheon, dinner, or formal meeting, a copy of the order of procedure should be emailed to firstname.lastname@example.org.
The form should be completed NO LESS THAN TWO (2) WEEKS PRIOR TO THE DATE OF THE FUNCTION in order to allow arrangements to be made for her attendance. When your request has been processed we will confirm the Mayor’s availability in writing. These rules also apply when requesting the attendance of council members.
